Service Description
This service includes a telehealth consultation, review of diagnostic questionnaires, scales and historical reporting from patient. No more than two animals are able to be considered. Upon approval, you will also receive a letter to present to your housing for your animal(s). This package also covers any required verifications from your housing provider. It is important to note, Support Animal Letters are not for sale. You must meet diagnostic criteria as set in place by HUD and the FHA; there may be potential that an animal is not recommended as part of your treatment plan. Should this be the case, we will discuss alternative options.
